Psychological Impact of Recurrent Pregnancy Loss: A Review of Emotional Trauma and Coping

Abstract

Recurrent Pregnancy Loss (RPL), which is a condition of two or more pregnancy miscarriages that happen in a row of 20-24 weeks of pregnancy is a serious reproductive and psychological problem. Although it has traditionally been considered as a medical condition with multifactorial etiologies, such as genetic, immunological, anatomical, and hormonal ones, recent findings point to its significant emotional consequences. The effects of RPL on women are often long-term grief, depression, anxiety, post-traumatic stress symptoms, and low self-esteem, with no social support and societal stigmatization. There is the psychological effect on relation to marriage, family relations and future pregnancies that tends to cause anxiety and emotional conflict as well as hypervigilance with regards to pregnancy. The coping strategies are diverse, and adaptive coping strategies like emotional expression, social support and cognitive reframing enhance resilience, and maladaptive coping mechanisms like denial, social withdrawal and avoidance further increase distress. Nurses and midwives will be at the center of reducing such effects by conducting early psychological diagnosis, expressive communication, educating patients, and referring them to counseling services. Cognitive-behavioral therapy, grief counseling and couple-based therapy are psychotherapeutic interventions that were proven to assist in emotional recovery and coping. However, even with the progress, there is the necessity of bridging the gap in culturally sensitive care, longitudinal assessment, and introducing the concept of mental health support to the conventional reproductive health services. The future studies must focus on discipline integration, digital mental health programs, and the establishment of a standardized screening protocol so that the entire support could be achieved. Understanding RPL as a medical and psychological phenomenon is a key to offering comprehensive care, resilience, and an improved quality of life of women affected and their families.
